0-0 always means castles K-side, Q-side is written 0-0-0. The 0s represent the empty spaces between the king and the rook, you see.  :D

R on d and K on c?

It's king on c1/c8 and rook on d1/d8. Incidentally, queenside castling is still legal even if b1/b8 is attacked (though the other two squares can't be).
